Consent to Release Information for Requesting Accommodations and/or English Learner Supports

Without parent or examinee consent to release information to ACT, they cannot process requests for accommodations authrized by ACT or English learner supports. The Test Accomodations Coordinator (TAC) is responsible for ensuring that the school has consent prior to submitting a request.

  1. Ask a parent, the examinee (if 18+), or school official with parental consent, to sign and date the Consent to Release Information to ACT form.
  2. Keep the signed release on file at your school.
